Enrollment Application Check List for Degree and Certification Application

NOTE: IT IS NOT NECESSARY TO ENROLL IF YOU WISH TO ORDER COURSES. WE DO RECOMMEND THAT YOU ENROLL AS A NON-DEGREE STUDENT AT NO CHARGE IF YOU WISH TO BE ASSIGNED A FACULTY ADVISOR TO ASSIST YOU. YOU MAY TRANSFER OVER TO A DEGREE OR CERTIFICATION STATUS AT ANY TIME.

Please fill out the appropriate application (Degree and Certification Application or Non-Degree Application / Home Study Application form and be sure to include the items on the check list below.

Application to either the Non-Degreed Adult Education Program or the Certification and Degree Programs need only be made once.

If you are enrolling in a self-study course, the home study enrollment form must accompany the appropriate CMS application and fees

______$30.00 one time administrative fee (US funds or credit card) for certification and degree students only.

______Degree and Certification Application including course material and handling fees.

FOR POSSIBLE CREDITS TOWARD YOUR DEGREE OR CERTIFICATION PROGRAMS

______College Transcripts and High School Diploma or equivalent.

______Notarized Resume

______Copy of notarized certificates of completion for seminars, workshops and courses attended. Have the number of hours of class time listed on each. (If you prefer, you can include a notarized cover sheet listing the seminar/workshop/course title and actual clock hours.)
